Occurrence Reporting Process

  • If you having issues within a facility, in the home or with VA+ staff, please call or email your direct supervisor with the details. You are also encouraged to fill an occurrence report
  • If your issue concerns your direct supervisor, contact COO or CEO as first line of reporting.
  • For all issues related to patient care, please complete an occurrence form found below.
  • You will receive confirmation of your report via email once it is submitted.
  • An administrator or supervisor will contact you regarding your concerns.
